Creating a Data Entry Form with Conditional Logic

Conditional Logic allows you to configure your form to show or hide field collections based on answers to form questions. This allows you to easily record only the information that is needed for a specific individual and quickly allows you to get to the needed fields and questions without scrolling through a full master form. 

To start, create your Data Entry Form in Tools > Data Entry Forms.

Next, remove any preset fields you don't need and begin adding additional fields and questions. Add in conditional logic to collect all the information you need from the specific people you're looking for. For example, let's say you're with an animal adoption organization that's tabling at an animal lovers event. You'd like to collect contact information on only animal lovers who you can later target and ask them to adopt an animal in need. Many people may fill out your form at the event, but by adding conditional logic you can set up your form so you can easily control what information each person is asked and collect contact information from only the people you're interested in contacting in the future.

Add your first question by choosing from a standard field type:

Since you're looking for people who'd be interested in adopting a pet you probably only want to ask that question of those who answer "yes" to "Are you an animal lover?" Keep that in mind as you add in the next question and add conditional logic to it. 

Once you've added in the question, for example, "Are you interested in adopting a pet in the next 6 months?" you'll be able to add in the conditional logic.

Click "add conditional logic" and then choose the first question in your form from the drop-down and select "yes" or "no." Once that logic is set you'll notice a crossed off eye icon in the upper left-hand corner of your second question that indicates that the second question will only be visible to the form collector if the individual answers "yes" to the first question.

Next, under the profile fields category, you can add in email and phone number fields and apply conditional logic to then only ask those who answer "yes" or "maybe" to your second question for their contact information.

Once you're done adding contact fields click "next" and then either save your draft or publish your form.

You will then be taken to the main page of your Data Entry Forms and the form you just created will be found at the top of the list. Click the edit gear to the right of the form to either fill out your form, edit, or delete it.

When you fill out your form you'll notice that your second question appears and changes based on your first response.

You can always fill out your form via the Data Entry Form tool or you can go directly to an individual profile and fill it out there. No matter where you fill it out, Crowdskout will automatically append the collected data to that profile.
